Office Policies

Your First Appointment

Bring your insurance card, driver’s license, and a current list of medications. Please come 15 minutes early to complete the intake forms unless you have already done so.

Referrals (HMO’s Only)

Check with your insurance company if you need a referral, then contact your primary care physician to obtain one. You’ll need to a referral at the time of your visit, or else your appointment will be rescheduled.

Lab Test Results

Lab test results are received within 7 days.  We will contact you for abnormal lab results and schedule a follow-up appointment.  We will not contact you if your lab results are normal.

Refills

The best time to ask for refills is during an office visit. If you need a refill outside of your scheduled time, make sure to call your pharmacy for refills or login to your portal. It may take up to three days to process medication refills, and if your insurance plan requires prior authorization, the delay might be a week.

No Shows and Cancellations

You will receive an automated reminder call and email for your appointment 7 days ahead. Cancellation calls less than 24 hours ahead are charged a $25 late cancellation fee.  A missed appointment will be charged a $25 no show fee. Two consecutive no-shows may result in dismissal from the practice.

Medical Records

If you are transferring to another physician, please contact them to fill out their form. If you are requiring your own records, fill out the Records Request and fax to 512-873-7878.  The charge is $25 for the first 20 pages, then .50 cents per page beyond that.

Insurance Plans

We accept most insurance plans with the exception of Medicaid. The best way to know if your providers take your insurance is to look at your plans “Provider Directory” and if they are listed, than they are contacted with that plan. You can also call your insurance plan to verify that Dr. Scumpia is in network. Any balance not paid by your insurance plan becomes your responsibility. Make sure you provide your insurance company any information it requests to process your claim.

Payments

Copay, coinsurance and deductible will be collected at check-in before your visit, and the entire balance for cash pay patients. We accept cash, check, Visa, MasterCard, Discover and American Express.
